Child Mental Health Services:

The Child Mental Health Program provides a comprehensive array of community-based treatment and support services to children with Serious Emotional Disturbances and their families that allow these children to remain in the safest environment, where they can reach their optimal potential.

 Clinic Treatment for Children and Adolescents - Serves children and adolescents with mild to moderate problems or mental illness and their families. Individual, group and family treatment is available based on the individual needs of the family. Psychiatric evaluations and medication clinic is offered as needed. Clinic case management services are available for assessment, planning, linkage/referral, monitoring, and advocacy. Case management services occur in the community.

Enhanced Treatment Center - This program is offered for families who need extra help to maintain their children in the home due to the child's emotional or behavioral problems. This level of care is appropriate for children who have a history of hospitalization or placement or are at-risk for placement. Individual, family and group treatment is offered along with psychiatric consultation and medication if necessary. Case management services are an integral part of the services delivered to the family. Treatment is oriented towards a team approach and attempts to assist the whole family as much as possible.

Enhancement Step Down/Diversion

This service is intended to provide a time-limited transition from

Family Voices Network (FVN) care coordination to natural supports and/or to divert a child from entry into the care coordination level of care.  A care coordination program and/or the FVN coordinator can make a referral to Enhancement Step Down/Diversion from the program's wait list or larger FVN wait list.  Care coordination agencies can make a referral for families ending care coordination but who need time-limited continuation of services such as respite, skill building, etc.

SCHOOL BASED SERVICES

School Based Mental Health Program - is a collaboration between New York State Office of Mental Health, New York State Education Department, Erie County Department of Mental Health and the United Way-Closing the Gap. The four components of school based mental health services are:

 

·      The integration of mental health supports and services into the regular education processes for identifying at-risk children, as well as into the processes for planning and delivering services to the popluation.

·      The development or enhancement of family support mechanisms within the school.

·    Training for school staff, families and mental health professionals in the areas of individualized care planning and strengths based approaches; behavior management; family/professional collaboration; recognizing the symptoms of childhood emotional and behavioral disorders, and effective strategies for addressing these disorders.

·      The provision of school based clinical services.

Family Voices Network (FVN):

Child Intensive Care Coordination- This program targets children and adolescents ages 4-18, and their families, through the Family Voices Network Wraparound process, who would benefit from the cultural discovery process consistent with wraparound values and principles.  All children enrolled in this program have a serious emotional disturbance.  All children and families participate in a strength and cultural discovery process in order to form a child and family team, comprised of paid professionals, community providers and natural supports.  The family identifies the members of their team.  The child and family team determines the supports and services required to meet the needs of the family, as well as help the family move from paid professional services to community and natural supports.  The Intensive Care Coordinator acts as the facilitator of the process.

Home and Community Based Services Waiver Program - Provides service coordination for children who are at the highest risk of out-of-home placement. In partnership with other child agencies, HCBS offers intensive support and monitoring through home visits, respite, skill building, crisis intervention and family support services. Individualized Care Coordinators are available 24 hours/day, 7 days/week. All services take place in the families' home or community.

OTHER SERVICES

Family Support Services - Is comprised of both formal and informal services that are determined, defined and delivered by families. Staff in this program has children who are currently receiving or have received services from a child mental health program. The family support program provides services to families with a serious emotional disturbance residing in Erie County. These services include information and referral, education, advocacy, respite and family recreational activities. Staff in this program also represents Mid- Erie in many community based task groups and committees. Referrals to the family support program can come from any agency or parent in Erie County.

Grandparent Advocacy Project- Emphasizes a team approach that will wrap services around the family to support the grandparents and relative caregivers. The team is comprised of a kinship services facilitator and an attorney from Neighborhood Legal Services. Families coming to the Court in crisis will be able to have immediate access to support and services. The team will walk a family through both the process and the system until their custodial decisions and arrangements have been made. They will also follow up with linkages to existing community resources and monitor that families have been able to access them. This program is housed at Family Court.
